New York City Department of Investigation (DOI) Background Investigation

Managers who are members of the management pay plan Level G5 or higher (regardless of civil service status) or employees working in an authoritative role as outlined by the Department of Investigation (DOI) Background Investigation Criteria , are required to undergo an additional, more complete background check managed by DOI.

Required Forms

Use the links to access and complete all the required DOI forms below. This is part of your initial on-boarding package.

You must complete, notarize where appropriate, and submit the documents to the Civil Service Management Unit (CSMU) within 30 days of your start date.

NOTE: New York City Department of Investigation (DOI) will determine when the fee of $250.00 will be processed through a payroll deduction. CSMU will notify you in advance.

Once completed and the appropriate documents have been notarized, all the paperwork including those listed on the DOI Package Checklist should be sent to CSMU in one email DHRDOIContact@schools.nyc.gov.

Safeguarding your Information 

As the authority on civil service and human capital professionals, we are obligated to safeguard employees’ confidential information. 

We highly recommend that you encrypt the email before sending to CSMU. The screen shot below illustrates the “encryption” button in Microsoft Office 365: 

The encrypt button is in the top banner on emails next to attach and discard.

Once received, we will password protect the background questionnaire and any documents DOI has deemed confidential prior to sending to DOI.

Important: Failure to submit the DOI Background Questionnaire and authorizations required, or to receive clearance by the New York City Department of Investigations, shall constitute grounds for denial of appointment, assignment or promotion to the affected position; or if employment in the position has already commenced, shall constitute grounds for termination of service.
